Trap lines are billed at $50.00 per month plus 10 cents per minute.

Monthly Auto-Debit
I hereby authorize Accurate Information Recovery Inc to charge my credit
card each month for our system usage. The first billing cycle will typically
begin near the 1st and / or the 15th day of each month, beginning with the month
in which the account was established.

Payment of Past Due Invoices
I also authorize Accurate Information Recovery Inc to charge my credit card
in the event that my account becomes fifteen (15) days or more past due.
(This option is only available to accounts that have been approved for Net 15
invoicing)
I understand that if my credit card information changes, such as an updated
expiration date or billing address change, I understand that it is my
responsibility to notify Accurate Information Recovery Inc of the change(s).
Failure to maintain a valid credit card on file, or provide on-time payment
of invoices may result in the suspension of services being provided and/or
pending results being withheld.

Calls from pay phones carry a 54 cent surcharge per call, mandated by the
FCC, which is passed on to you at cost. Multiple cardless calling card calls can
be made with only one surcharge. Storage for 2GB of audio and fax media, and
bandwidth transfer of up to 20GB per month is included with your product.
Excess storage and/or bandwidth usage will result in an additional charge of
$10.00 per month for each additional 2GB / 20GB block used. Monthly charges
will be automatically deducted from your credit card . Call charges will be
deducted from your credit card once your usage has reached 75% of your activation
fee.

It is the customer's responsibility to maintain sufficient balance on credit
card account to allow settlement of charges. Interruption of service will
be experienced in the event that credit card account balance or prepayment
balances are not maintained. On hold accounts may be charged a $20 reconnect
fee. Returned checks will result in a $25 returned check fee being applied to
your account. Unpaid invoices are charged 1.5 % interest per month.

It is specifically understood and agreed that the purchaser shall be using
this service solely for lawful and appropriate purposes and the purchaser
hereby agrees to indemnify and hold Opperman Investigations and Accurate
Information Recovery Inc harmless from any and all claims of any nature whatsoever
arising out of or concerning purchaser's use in any manner of the services
provided herein. In the event that Opperman Investigations and Accurate
Information Recovery Inc is brought into any action arising from or concerning
purchaser's activities, purchaser agrees to indemnify and hold Opperman
Investigations and Accurate Information Recovery Inc free from all arbitration, court and
attorney's costs and fees.

Opperman Investigations and Accurate Information Recovery Inc reserves the
rights to immediately discontinue, disconnect, limit, or revoke service without
warning to any customer who, at the sole discretion and determination of
Opperman Investigations and Accurate Information Recovery Inc, causes any type
of activity or load which is incompatible with our network, causes quality of
service issues, or otherwise impairs our ability to maintain our services and
provide service to our customers. Opperman Investigations and Accurate
Information Recovery Inc shall not be liable for any direct or indirect damages
resulting from a decision to discontinue, disconnect, limit, or revoke
service with or without warning.

This Agreement shall be governed by the laws of the State of Colorado and
any disputes or controversy arising hereunder shall be arbitrated or adjudicated
in City of Denver, State of Colorado. Purchaser hereby consents to personal
jurisdiction for all claims of any nature concerning purchaser and Opperman
Investigations Inc, California and specifically consents to service of process
being effectuated by certified mail at purchaser's address. In the event
that any action is brought to enforce or construe any term or condition of this
agreement, or for the breach of this Agreement, or concerning the
indemnification clause contained herein, the prevailing party shall be entitled to
recover, in addition to all other damages, reasonable attorney's costs and fees.

Service although reliable is not guaranteed. Liability for interruption of
service and/or a lack of quality of service including but not limited to errors
and omissions relating to directory listing and CNAM shall be limited to a
maximum of two months voice systems rental. We reserve the right to interrupt
service for maintenance and systems upgrades at our discretion.

It is specifically understood and agreed that the format of services
provided and all programs associated with the services provided under this agreement
are Opperman Investigations and Accurate Information Recovery Inc
proprietary materials and information and that purchaser shall not, without the prior
written approval of Opperman Investigations and Accurate Information Recovery
Inccopy, duplicate, communicate or modify format of services provided and all
programs associated with the services provided under this agreement

Service is month to month and may be canceled at anytime upon 30 days
written notice.

If paying by credit card and your card is charged successfully, in most
cases, you will be set up with your toll-free number, PIN and instruction guide
immediately.
